Disconnects can happen for a variety of reasons. Could be something on your system (Flash, streaming video/music, other processes can also cause issues). Could be one of any number of hops between you and the Live server. Could potentially be the server too.
That said, if you are on the app, in particular on iOS, there may actually be something more going on. I've seen a number of topics with disconnects. Can't say if it is a new iOS issue, something with the app code, or something else entirely.

Turn off "always show from white's perspective" in settings.
The same thing happened to me. I won, but the app stalled so I didn't get points

I am a bit disappointed of the android app the last months. Chess updates it quite often, but it's getting more and more buggy. Private messages are not visible since weeks, also the quite new introduced compare stats with another player shows no results anymore....handicap games the manual settings are also invisible ....since an update last year, games are not updated immediately ....chat is not possible after doing a move for a time or if oppenent moves while writing (message "please refresh the game" and message is not sent ). Several times per day chess is not available, although my connection is stable and fast. The design improved a bit , but many website features are still missing (no variants, an info box with more game information like on website, tournaments are not reachable with a single click). Some features I read were also requested by many other users for awhile , the app is developed very slowly and when I report bugs, they are not fixed quick. ....

Version 4.5.0 should be released in the near future that is supposed to fix the messages issue. As to features, such as variants, most variants are unlikely to work well on the app due to limited screen real estate. The apps are also not designed to be in 100% feature parity to the website and mobile web works well for most things.

I can’t do anything, it’s frozen. It tried to update at first, then stopped doing that.

App customization may be, not sure how iOS handles that. However, all data is hosted on the site and the apps just load it from there.

Checkmate on the client doesn't necessarily mean the server received your move before the clock ran out. The server keeps the official clocks and if your move isn't received before time runs out, it will be a loss if your opponent had sufficient material to mate.
I find mistake after mistake on chess.com
It used to be excellent but not any more.
