No, it is from Tactics training. As it is a difficult puzzle, I will 100% fail in first attempt of puzzle rush.

Why > 95% fail rate?

Cos they just put every new puzzle as 800 rating. So more than > 95% of low rated people  failed it and the puzzle rating shoot up from 800 to 2600.  But I guess there will be 30-40% minimum pass rate among 2500+ rated people.

How does a puzzle with a 0% pass rate even have a rating?

Very easy puzzle, I get within 15 secs. 1. Qg6, fxg7, 2.  Nf6+ Kh8 3. Mg6+, Mate in 3. 

Puzzle rating , players rating around  1100- 1500 with 0%  is not surprising. 

The site assigns an initial rating and adjusts it using the normal rating formula when attempted.

A puzzle with multiple moves can be failed on any of those moves and a number of people may fail after getting the first move right. So, a high rated puzzle with no pass rate likely doesn't give partial credit, or it takes multiple moves for partial credit, and players are missing a later move. A low rated puzzle with a low pass rate gives partial credit and people are getting some moves right, which lowers the tactic rating, but ultimately fail the rest of the puzzle, which drops the pass rate.
