Shucks, the ARB Chess system troll is back. Look at his only two topics and his zero chess games if you don't believe he is a troll.
The answer to all of these problems is empathy. It is the ability to put yourself in another person's shoes, to really understand their feelings and motivations. And even if you don't agree with the behaviors and conclusions, to at least validate that the feelings are real.

It's estimated that 5% of world's population are psychopaths/sociopaths, so you are on a hiding to nothing in personal appeals like this. I very quickly turned off all possible personal interaction facilities - so people can't attack me when playing a game, or post nasty notes. I do look at othe people's notes, and post nice ones whenever possible.
But I see some really nasty stuff, like a sexist/racist attack on a young lady yesterday. I suggested that she should report this to the moderators and almost did my so myself ... I should have ... If there was a "report abuse" button like there is on Amazon, then I'd have used it.
So why not have such a button?
Your three strike policy is too lenient, an immediate ban is called for, and not just of the user name, but of the email address they use, making it as difficult as possible for the psychopath to get back on.
Maybe you can have a sub domain anythinggoes.chess.com where the psychopaths can be psychotic together? That might stop them trying to get back on the main site, as well as giving them what they want.
There is an obvious solution to the too much/not enough moderation debate. Simply give each member the ability to control whose posts they see, as is done on Facebook. For example, if I find the posts of ObnoxiousGuy999 to be offensive, I could add him to my private do-not-see list. Then I would no longer see any of his posts (until I took him off my list). I would instead see a blank space or similar place-holder associated with that post number (preserving the post numbering). Every other chess.com member (that hasn't yet added him to their list) could still see all of ObnoxiousGuy999's posts. Initially, upon implementation of this system, every member's do-not-see list would be blank -- we could all see each other's posts.
This appraoch would simultaneously allow maximum freedom of speech, and personalized filtering. And the moderators would have a much smaller work load (still needed for filtering out blatant hate-speech and unapproved advertisements).

This doesn't protect the young and vulnerable suffuiciently, and I'd get sick of banning the obnoxious from my personal space. The moderator should just ban him if he's flagged once, and agrees that he's obnoxious (or divert him to anythinggoes.chess.com) If he doesn't think he is obnoxious then, of course, you will still be able to ban him from your personal space.
Agree with Turbofish's proposal. Simple, easy to program, doesn't not affect other people's views, less work for moderators.
Kinda like what the block function should be doing: you should not see the person you are blocking anywhere on the site (even in the ratings lists in live chess). Personalised blocking makes it happy for everyone.

Submit the button suggestion in the Help and Support Forum. I'm not so sure banning is so effective. Short term, it removes the offender, but keeping them out isn't so easy.

As far as making the forums better, whether here or anywhere on the internet, I just don't know how a rotten egg can be made "better".
